* Hard minimum malpractice base of £950
* Excess options: £0, £250, £500, £1,000
* Limit options: £1m, £2m, £5m, £10m (no £3m)
* ERP is a multiplier (5y=1.00, 10y=1.225)
* GDC is always included, flat by turnover band
* Practice owner cover: per-staff-type rates
* Multipliers only apply to malpractice portion
